This home study can be accessed and read by individuals for free. If you are from a TCHP Consortium hospital, your contact hours for completing a home study are processed without charge. Your post-test and certificate of completion will be returned to you through your hospital's education department. It cannot be mailed to your home. If you have signed up to take the class that has the same name as this primer, you do not need to mail in your post-test. Bring your post-test and evaluation form to class with you and the program coordinator will process it for contact hours and return it to you with your certificate of completion. If you are not from a TCHP hospital, the cost for processing this home study for contact hours is $10. Your post-test and certificate of completion will be mailed back to you. If you have signed up to take the class that has the same name as this primer, the cost of the primer is included in the course tuition. In this case, you do not need to mail in your post-test. Bring your post-test and evaluation form to class with you and the program coordinator will process it for contact hours and return it to you with your certificate of completion. Please be aware that this is copyrighted material and can only be used by an individual. If you duplicate the home study for others without obtaining permission from TCHP, you are in violation of copyright law. Once you have read the home study, complete the post-test and evaluation (the last 2-3 pages of the home study) and mail them to TCHP with the appropriate fee (if applicable--read first 2 paragraphs above).
