Returning Your Post-Test to TCHP

There are several ways that you can submit your post-test to TCHP:

• BEST WAY: For electronic submission, return to the main home study page and click on the post-test for the home study you are completing. Fill in your answers, print a copy for yourself, then hit the "submit" button to send the post-test to TCHP. You could also save your filled-in post-test to your desktop and then email it as an attachment to tchp@hcmed.org
   

• For submissions filled out by hand:

-Scan your post-test and email it as an attachment to tchp@hcmed.org
-FAX it to 651-225-2980
-Mail it to:

Please keep a copy of your post-test for your records; it will not be returned to you.
